About this reference

About this reference

Titration Ledger is an independent study reference for the BCPS exam. It is published by the team behind Pharmacotherapy Exam Prep 2026, a study app for iPhone and iPad, and the Get the app buttons on these pages lead to that app. The site has no connection to the Board of Pharmacy Specialties (BPS), to Pearson VUE, or to the American College of Clinical Pharmacy (ACCP) and the American Society of Health-System Pharmacists (ASHP), and no page here speaks for any of them.

1Where each kind of content comes from

Exam facts
Fees, timing, scoring, eligibility and results come from BPS documents: the BCPS examination specifications (effective September 2024), the Candidate’s Guide, the dates and fees page, the scoring page and BPS’s results releases. Each fact page names its source and the date it was checked.
Domain weights
BPS publishes weights for three domains (36/36/28) and their subdomains. Weights for single disease areas, such as cardiology, are our estimate from BPS’s 2023 job-analysis ratings, and every page that shows one labels it as an estimate.
Clinical content
Tied to a named guideline and year, such as the 2022 AHA/ACC/HFSA heart failure guideline, the ADA Standards of Care in Diabetes 2026, GINA 2026, GOLD 2026 and the AGS Beers Criteria 2023. Where a guideline changed in 2025 or 2026, the page teaches the current text, says what changed, and does not guess which edition the exam follows.
Case rounds
Study cases written by our editors for practice. They are not BPS items; the BPS sample items are linked from the BPS site, never copied.
Pay data
Only US Bureau of Labor Statistics figures for pharmacists, because BLS does not report board-certified pharmacists separately.

2How a case is checked

  1. Solved without the key

    Before its option notes are written, the case is answered by a reviewer who sees only the stem and options.

  2. Compared with the key

    A case where that answer and the key disagree is held back, rewritten or replaced.

  3. Checked against the source

    Cases tied to a guideline that changed recently are matched to the edition they cite, and each case is placed under the outline area its topic belongs to.

  4. Annotated option by option

    Every option gets its own note on why it is right or wrong.

3What this site does not do

The site keeps to study material, so it does not:

  • speak for BPS or describe any relationship with it;
  • reproduce BPS exam items or the BPS sample items;
  • predict your score or your result;
  • give clinical advice: for patient care, use the current guideline, the product label and your institution’s protocols;
  • publish prices for prep products it could not verify.

4When something here is wrong

BPS updates its fees, outlines and policies, and guidelines change between our checks. If a page no longer matches the BPS document or guideline it cites, tell us through the contact page; until the fix is live, the source document is the one to follow.

To start studying, the BCPS study guide lists every domain and concept page on the site.
